Print a permission slip

At many of our activities, we ask young people to come along with a completed permission form signed by a parent/carer. This makes sure we have all the information we need to keep people taking part in our activities safe and up to date with the latest information about their groups and activities.

Permission forms are valid until October the following year after they are complete, and must be completed and returned before young people are able to attend trips or residential events.
